Goal system
Previously, caretakers from each organization relied on disorganized methods using paperwork, mail, and phone calls to request information.
These methods limited and slowed access to critical information that is necessary for the care and placement of foster youth.
Co-Creation Activities
Schedule Mapping
8 HCJFS users and 10 Cincinnati Children’s users participated in an open-ended journaling activity that conveys to us what tools they currently use in their position and identify moments during which they would use the portal.
Content categorization
The stakeholders participated in a card sorting activity that sorted patient information in order to inform the structure of the site and hierarchy of information.
Information Architecture
Based off of research from co-creation activities, interviews, and observational research, we mapped out the structure of site.
Wireframing
We brought printed wireframes to our sessions with stakeholders in order for the users to test workflows and features. Our stakeholders prioritized elements that felt most intuitive and functional to them including multi-search fields for finding the correct youth’s data, bookmark navigation for frequent searches, and on-screen navigation to quickly move through information.












Prototype Testing
15 users from both organizations tested our designs using an interactive prototype.
Final Design
MODULAR + INTERACTIVE + ACCESSIBLE
The brand for Identity emphasizes the goal of togetherness and quality of care. The website features a modular design to give users agency using collapsible fields of information, interactive elements including alerts and an interactive calendar, and an accessible design that suits a range of users from clinicians to caseworkers.
Identity was piloted in 2018 for use at Hamilton County Job and Family Services. As of 2020, Identity has expanded across Ohio and shapes the national approach to data sharing between social work and medical services.
